23-24 Sports Physicals

Once again, Sangamon Valley is offering students and families a great opportunity to get ahead of needing sports physicals for the 2023-24 sports seasons. On Tuesday, May 23 (2pm dismissal) from 2pm-5pm. SVHS is hosting the SIU School of Medicine, as well as medical staff from ORSCI office to provide sports physicals for incoming 7th, 8th, 10th, 11th, and 12th graders (6th and 9th graders still need school physicals from primary care providers).

To take advantage of this opportunity, please fill out the IHSA/IESA physical form (link below) and bring it with you for your physical. There is a cost of $35, cash or check is accepted (make check payable to ORSCI). No insurance or medical cards will be accepted.
